Enrollment Information
If you need to register a new student for the upcoming 25-26 school year, you may complete the forms below, or stop by the NLMS office and pick up a paper registration packet.
1. Fillable Enrollment Packet (Also available in Spanish, Russian and Ukrainian)
2. 6th Grade Registration Form or 7th Grade Registration Form
3. New Student Enrollment Questionnaire
Documents needed in addition to the registration forms include:
1. Birth Certificate
2. Immunizations
3. Proof of Residency (Purchase, Lease, Rental agreement or current utility bill such as PUD)
Please note: Your address determines which school your child will attend. If unsure of what school that will be, please visit: School Site Locator and enter your address. The site will provide which elementary and middle school your student will attend.
Please note: If your child previously attended a private school, or a school outside of the State of Washington, please also complete an Authorization to Release Records Form.
You may email the completed forms and documentation to: Veronica Ainsa@lkstevens.wednet.edu or drop them off at the NLMS office. Once received, the Registrar will contact you. Questions? You may call NLMS at: 425-335-1530. The office is open from 7-3:30 Monday-Friday.
