New to the LSSD District?

Please complete the information below for new 6th grade students for the 2023-2024 school year.

**New to the District.......Please complete the following items:

1. Fillable Enrollment Packet (English)

2.  7th Grade Enrollment Form

3.  New Student Enrollment Questionnaire

Documents needed in addition to the registration forms include:

1.  Birth Certificate

2.  Immunizations

3.  Proof of Residency

Please note: If your child previously attended a private school, or a school outside of the State of Washington, please also complete an Authorization to Release Records Form.
